Health Savings Accounts

Capital Bank can now offer Health Savings Accounts to you and your employees. A Health Savings Account (HSA) is an account that you can put money into to save for future medical expenses. There are certain advantages to putting money into these accounts, including favorable tax treatment. You can use the money in the account to pay for any "qualified medical expense" permitted under federal tax law. This includes most medical care and services, and dental and vision care, and also includes over-the-counter drugs such as aspirin, contact lens solutions, as well as COBRA payments, deductibles and other qualified expenses etc.

Below is a list of forms you may download to establish an HSA with Capital Bank. (Please do not email these completed forms to the Bank as that does not protect the confidentiality of your information.) You can return these completed forms by mail or contact us to find out more information about opening an HSA account.  If you return these forms to us in the mail, please also include a copy of a valid driver's license or other government issued, picture ID.
